- This contribution is made from my own funds, and funds are not being provided to me by another person or entity for the purpose of making this contribution.
- I am a U.S. citizen or lawfully admitted permanent resident (i.e., green card holder).
A copy of our report filed with the State Board of Elections is (or will be) available on the Board's official website (elections.il.gov) or for purchase from the State Board of Elections, Springfield, IL. State law requires political committees to use their best efforts to report the occupation and name of employer for each individual whose contributions aggregate in excess of $500.
